Processing of (special and/or sensitive) personal data of clients We process personal data to provide care, make appointments, for administrative purposes and to communicate. This concerns first name, middle name, last name, date of birth, gender, address, telephone number, e-mail address, health insurance details, citizen service number (BSN), health data, other personal data that you actively provide, for example in correspondence or by telephone.
Data from persons under the age of 16 - our website and/or service does not intend to collect data about website visitors who are younger than 16 years old. Unless they have permission from parents or guardians. However, we cannot check whether a visitor is older than 16. We therefore recommend that parents be involved in the online activities of their children, in order to prevent data about children being collected without parental consent. If you are convinced that we have collected personal data about a minor without this permission, please contact us via info@chiropractieoosterheem.nl, then we will delete this information.
We use the Cliniko system to make appointments and manage client data . Data you enter is stored on secure servers and processed according to Cliniko's privacy conditions. Contact forms on our website are sent directly to our e-mail inbox . By making online appointments, you agree to Cliniko 's privacy conditions.
Our website only uses technical, functional and analytical cookies that do not infringe on your privacy. These are necessary for the functioning of the website and your ease of use. You can disable cookies via the settings of your browser, but this may affect the functionality of the website.
We process your personal data exclusively for the following purposes::
We are required under the Medical Treatment Agreement Act (WGBO), the Health Insurance Act ( Zvw ) and the Use of Citizen Service Number in Healthcare Act ( Wbsn-z ) to establish your identity, maintain a medical file and register health insurance details. For this purpose we need, among other things, your name, date of birth, BSN, proof of identity and insurance details. If you do not provide this information, we will unfortunately not be able to treat you, because we would be acting in violation of applicable legislation.
We believe it is important to protect your data and have therefore taken organizational and technical measures. If you have the impression that your data is not properly secured or there are indications of misuse, please contact info@chiropractieoosterheem.nl.
You have the right to view, correct or delete your personal data. In addition, you have the right to withdraw your possible consent for data processing or object to the processing of your personal data by our company and you have the right to data portability. This means that you can request us to send the personal data we have on you in a computer file to you or another organization named by you. To make sure that the request for inspection is made by you, we ask you to send a copy of your ID with the request. In this copy, black out your passport photo, MRZ (machine readable zone, the strip of numbers at the bottom of the passport), passport number and Citizen Service Number (BSN). This is to protect your privacy. Chiropractie Den Haag/Oosterheem will respond to your request as soon as possible, but at least within four weeks. Personal data are not retained longer than necessary. We retain medical records for 20 years in accordance with the guidelines of the Dutch government. We retain invoice data for at least 7 years in connection with tax obligations.
We do not make decisions based solely on automated processing that could have (significant) consequences for people. These concerns decisions that are made by computer programs or systems, without a human being involved (for example an employee of Chiropractie Den Haag/Oosterheem).
Personal data will only be provided to third parties if this is necessary for the performance of the service or if we are legally obliged to do so. We use third parties such as Cliniko for file management, Free Motion Physiotherapy for care cooperation and Mailchimp for sending newsletters. Processor agreements have been concluded with all these parties to ensure the security of your data.
Mailchimp , a service of Intuit Inc. located in the United States, is used to send newsletters and marketing communications. Your name and email address are provided by us to Mailchimp and processed on their secure servers. Mailchimp keeps statistics such as open and click behavior to improve our communication. You can always unsubscribe via a link at the bottom of the newsletter. The data transfer to the USA takes place on the basis of standard contractual clauses in accordance with the GDPR.
